This form should be used to request cancellation of services billed to a PageTiger Central Group Account. Once processed, any cancellation will apply to all PageTiger Accounts and Users within the Customer Group.
The completed form must be submitted at least thirty days in advance of the corresponding subscription to which the cancellation request applies.
